Care Coordination

In partnership with Trilogy Care, guardians & co. Care Coordinator will facilitate services and supports for our clients, working closely with families to manage your Home Care Package Plan

What we do:

  • Develop goals and individual care plans in collaboration with families.

  • Facilitate access to clinical services and health practitioners.

  • Coordinate other support service providers, contractors upon request.

  • Maintain clinical records and writing reports.

  • Track performance of recipient care plans.

  • Ensure compliance with relevant legislation and guidelines.

  • Roster support services and staff members.

  • Advocate for recipients.

  • Weekly Check-in via phone call or home visit if preferred.
